☑ You have discussed this with your club president

☑ You have discussed with the other team

☑ You have a plan for the reschedule

By submitting the form, you acknowledge that the request to reschedule is not final until you receive confirmation from Bunbury Netball Association. The committee reserves the right to deny requests – failure to adhere to the original fixture will result in a forfeit.
